Preferred QualificationsRole PurposeProvides direct patient care activities to home care or home health patients according to physician orders in the home environment including assessment, evaluation, treatment planning, implementation and instruction/supervision of physical therapy practice. -Responsibilities
      • Instructs and assists patient to perform various physical activities and in use of assistive and supportive devices such as crutches, canes, and prostheses.
      • Instructs patient and family in treatment procedures.
      • Re-evaluates patient and adapts treatment program accordingly to achieve maximum benefit.
      • Confers with physician and other practitioners to obtain additional information, suggest revisions in treatment plan and to integrate physical therapy treatment with other aspects of patients' care.
      • Evaluates, fits and adjusts prosthetic and orthotic devices and recommends modifications to Orthotist.Minimum RequirementsEducation
      • Bachelor's Degree - Physical Therapy (PT)Experience
